Form emails directed by user selection

Hello,

We are fast approaching the launch date of our new library website (http://library.uic.edu), and I've run into a small problem regarding a couple forms.

Based on what I learned at the WUC in last year, I was hoping to utilize a checkbox list on a couple forms to direct the form email to the proper address.

For example, here: http://library.uic.edu/home/services/instruction-and-workshops/request-instruction

I am using a data form (with "mail data" set to "yes"). I do not have any email address in the "To" field, which is hidden. Instead, in the field "Library, Subject Area" I have the following as possible values:

lib-class@uic.edu|Daley Library, Humanities & Social Sciences
klassen@uic.edu|Science Library, Sciences (including Math & Engineering)
lib-cref@uic.edu|LHS-Chicago, Health Sciences
lib-pref@uic.edu|LHS-Peoria, Health Sciences
lib-lhsr|CLHS-Rockford, Health Sciences
lib-uref|LHS-Urbana, Health Sciences

The idea being that the form data will get emailed to the proper address based on the selection.

Unfortunately, this doesn't seem to be working. My forms which utilize the "To" field to go to one default address are working fine.

I would really appreciate any insights the community might have! I'm trying to avoid building six separate forms for essentially the same data. 

By the way, we are running WebGUI 7.5.27.

My apologies, I realized exactly what I was doing wrong after submitting this question. My selection values needed to be in the email "To" field, not in a newly created field.
