For the past 2 to 4 releases the file field has been broken in Safari.  It's not always broken, but I'd say it's broken on 3 out of 4 CS posts, as well as in user profile editing.

Here's a screen shot.

Had to upload this in Firefox cuz it was broken in safari.

I've checked in a possible fix for this.  I can't reproduce this, so I can't verify that the fix will work.
